Why Male Factor Infertility is Underdiagnosed
In Indian society, infertility has historically carried an unfair and inaccurate female-only stigma. Biologically, male factor parameters are the sole cause in approximately 30% of couples and a contributing factor in another 20%—meaning half of all fertility journeys involve the male partner.
Early, comprehensive semen testing prevents female partners from undergoing months of futile, invasive hormone treatments when the primary underlying obstacle is paternal.
WHO 6th Edition (2021) Semen Analysis Normal Reference Limits
A reliable semen analysis must adhere strictly to the World Health Organization (WHO) 6th Edition guidelines, evaluated after 2 to 7 days of sexual abstinence:
- Semen Volume: ≥ 1.4 mL
- Sperm Concentration: ≥ 16 million sperm cells / mL
- Total Sperm Count per Ejaculate: ≥ 39 million sperm
- Total Motility (PR + NP): ≥ 42%
- Progressive Motility (PR): ≥ 30% forward-moving sperm
- Normal Morphology (Strict Kruger Criteria): ≥ 4% normal oval forms
- Vitality (Live Cells): ≥ 54%
Beyond the Routine Count: The Sperm DNA Fragmentation Index (DFI)
A man can possess a "normal" sperm count of 50 million/mL and 60% motility, yet suffer from severe occult male infertility due to fragmented DNA.
Sperm DNA fragmentation refers to single- or double-strand breaks in paternal genetic material, predominantly induced by oxidative stress, smoking, scrotal heat exposure, varicoceles, or systemic inflammation during the 72-day spermatogenesis cycle.
- DFI < 15% (Optimal): High reproductive integrity; favorable natural and IUI conception odds.
- DFI 15% – 25% (Fair): Moderate fragmentation; can often be reversed with targeted oral antioxidant regimens (CoQ10, L-Carnitine, Zinc, Lycopene) over 3 months.
- DFI > 25% – 30% (High): Significantly elevated risk of failed fertilization, Day-3 embryo arrest, and first-trimester recurrent pregnancy loss. In such cases, surgical sperm retrieval (TESA) or advanced microfluidic sperm sorting is strongly recommended.
